PMOS Case Study: How Anna Restored Natural Periods and Optimised Her Fertility
Anna in her 30s approached me with a previous diagnosis of PMOS, her 2 main goals being:
- Natural regular periods without the drugs she had previously been prescribed by her GP which had made her feel very unwell
- Weight loss which she had found difficult to achieve by herself
After completing a 5-page questionnaire including a 3-day food and lifestyle diary & getting some basic blood tests via her GP, I met with Anna and agreed a nutrition plan based on her Vegetarian preferences, the key challenge to ensure sufficient daily protein intake to promote weight loss and supplements to help stimulate monthly periods
The first 3 months were focused solely on restoring her periods & making sure the changes to her diet had become habitual. (Anna had a demanding job so too much change at once was unrealistic).
Whilst weight loss was slower than Anna would have liked, after 3 months she was having regular periods which she was delighted about!
To boost weight loss we agreed an exercise schedule, increasing daily activity levels (including strength training 2 x a week) but being careful to include rest & recovery days as too much stress on the body can raise cortisol & be counterproductive for weight loss.
In month 6 we agreed Anna would do a Dutch test to more accurately assess her hormone status as her goal now also included fertility.
The Dutch tests revealed a need for faster oestrogen clearance, a need to boost lower than “normal” progesterone levels (progesterone is key in fertility) as well as some nutrient deficiencies for hormone balance which could easily be rectified.
Whilst Anna continues to optimise her hormone & metabolic status through diet and exercise, we are exploring options for boosting her progesterone levels with Body Identical Progesterone to increase her fertility.
This case study reflects the complex nature of PMOS and how the evolving needs of a client depending on improvements in their hormone status ( in Anna’s case restoring regular period naturally), the challenges of affecting their metabolic status ( in this case Anna’s slower than merited weight loss) & their changing goals means that working with clients with PMOS is not a quick fix that can easily be gleaned from Instagram or ChatGPT but requires patience, nuance and an informed supportive approach.
